If you are a new patient, you will be asked to come about 15 minutes early to fill out some paperwork before your appointment. If you would like, download and print the new patient forms to fill out before you come to our office to save some time.

What Will Happen On the First Visit?

Please provide us with the following information to assist us at the time of your first visit to the office.

  • Your referral slip and x-rays from your referring dentist.
  • If you have dental insurance, please bring insurance cards with you to the appointment.
  • Medical conditions and medications list you are currently taking.

Please notify the office if you have a medical condition or concern before any treatment (e.g. artificial heart valves or joints, required pre-medication, severe diabetes, or hypertension).

We will complete an in-depth medical and dental health history and a thorough examination to test for endodontic disease. CBCT will be taken if needed.
